Some of these surgeries can be pre‑booked. Between 8.00 and 9.00am a sister will be available to speak to you on the telephone for any advice you may require. Also, between 2.00 and 3.00pm, a sister will be available for results of blood tests, smears, x-rays etc.

 

GENERAL MEDICAL SERVICES

All practices are expected to provide the following essential services:
Management of patients who are ill or believe themselves to be ill with conditions from which recovery is generally expected, for the duration of that condition including relevant health promotion advice and referral as appropriate, reflecting patient choice wherever practicable.
Management of chronic disease in the manner determined by the practice in discussion with the patient.
General management of patients who are terminally ill.
Practices are expected to provide continuous holistic treatment and care for all registered patients.

  • Contraceptive services
  • Vaccinations and immunisations (non-childhood)
  • Childhood vaccinations and immunisations
  • Child health surveillance
  • Maternity services (excluding intra-parturn care)
  • Minor surgery procedures such as curettage, cautery, cryocautery of warts and verrucas and other skin lesions.

Any patient over the age of 16 who has been registered with the practice for three years or more, can request a consultation and the practice, without prejudice to its other obligations in respect of that patient, will provide such a consultation. Any patient over the age of 75 can request a consultation and the practice, without prejudice to its other obligations in respect of that patient, will provide such a consultation.

REGISTRATION PROCEDURES

If you wish to register with Linden Hall Surgery, please either bring your medical card with you or ask the receptionist for a medical form GMS1 to enable you to register with the practice.

When registering at the practice you will be asked by the reception staff which GP you would ideally like to be registered with. We will endeavour to comply with your preference unless a certain GP has asked for no further patients to be registered with him or her for the time being due to increased workload.

At this practice, a patient can opt to see any GP of their choosing provided they are available at the time of request.

Antenatal – Asthma - Child Health Surveillance – Diabetes - Three Year Checks - Coronary Heart Disease - Well Woman - Family Planning - Immunisation Minor Surgery

DISABLED FACILITIES

We are acutely aware that the facilities at our main surgery are not entirely suitable for wheelchair users as we have an upper floor. However, doctors are more than happy to see a patient in a downstairs room if the patient makes this request known to the receptionist when they book in for their appointment.

OUT-OF-HOURS SERVICE

Outside normal surgery hours are covered by NHS Direct and their telephone number is 08450 20 2131. PLEASE MAKE A NOTE OF THIS VERY IMPORTANT NUMBER so that it is easily obtained when required in an emergency situation. This service is used every evening from 6.00pm until 8.00am the following morning and from 8 am on Saturday until 8.00am. on the following Monday morning. When using this service you may be requested to attend at the Princess Royal Hospital to see a doctor from the ShropDoc Co‑operative. Alternatively, they may offer you advice on the telephone.

HOME VISITS

Please remember that the doctor can see four patients at the surgery in the time it takes to do one home visit, so please try to get to the surgery if you can. However, if you are seriously ill and cannot come to the surgery, you can request a visit by telephoning 01952 820400 or 01952 670060 before 10.00am. The receptionist will ask for details of your illness to help the doctor.

REPEAT PRESCRIPTIONS

The majority of our repeat prescriptions are now computerised. On the right side of the prescription is a duplicate of the items you may re‑order. Please tear this off and keep it until you need to re-order. You can then either post it or deliver it to the surgery, clearly ticking which items you require. Alternatively, the local chemists will deliver your slip to the surgery on your behalf. You may also fax your request on 01952 825149 or 01952 606123.

COMPLAINTS PROCEDURE

Linden Hall Surgery has a complaints procedure in operation whereby if you have a complaint regarding anything you see or hear in the practice you may feel free to contact Mrs Brotherton, the practice manager, directly. Alternatively you may put your complaint in writing and this will be dealt with in a professional manner by Dr Lisk and Mrs Brotherton.

RIGHTS AND RESPONSIBILITIES OF THE PATIENT

The doctors and nurses endeavour to keep to time when conducting their consultations. Unfortunately, due to unforeseen circumstances, these will occasionally not run as they would wish. We do, however, expect patients to be on time for their appointments and also to show tolerance when they are occasionally kept waiting. The reception staff will always try to keep patients informed of any delays which may occur.

VIOLENT AND AGGRESSIVE PATIENTS

Violent and aggressive patients will not be tolerated in the practice, as the safety of other patients is paramount. Offending patients will be asked to leave the premises and will be taken off the list of their registered doctor.

DATA PROTECTION ACT

Under the Data Protection Act, any disclosure of information regarding a patient is not allowed without the patients' written consent. Even the fact that a patient has attended the practice will not be disclosed (eg to relatives, friends etc).

FREEDOM OF INFORMATION - PUBLICATION SCHEME

The Freedom of Information Act 2000 obliges the practice to produce a Publication Scheme. A Publication Scheme is a guide to the 'classes' of information the practice intends to routinely make available.This scheme is available from reception.
